UNDERSTANDING THE MEDICINE

Why ketamine?

Unlike traditional antidepressants, which can take weeks to show effects, ketamine can provide relief within hours for some people. It is believed to work by enhancing neuroplasticity—helping the brain form new connections and pathways—while also modulating the glutamate system, which plays a key role in mood regulation. At therapeutic doses, intramuscular (IM) ketamine can induce a non-ordinary state of consciousness, which may facilitate emotional processing, insight, and psychological healing.

Integration & lasting change

Integration means reflecting on the experience and applying insights in daily life. Dr. Canning strongly encourages an established relationship with a therapist, counselor, or integration coach with whom he can collaborate.

We can refer you to an integration specialist if necessary!

Integration in ketamine and psychedelic medicine refers to the process of reflecting on, making sense of, and applying insights or experiences from the medicine session into one’s daily life for lasting psychological, emotional, and behavioral change.

“The framework Dr. Canning created for my psychedelic experience struck a remarkably thoughtful balance. He offered the knowledge, skill, and reassurance necessary to support safety, paired with a level of humility that enabled my own exploration into potentially uncertain mental territory. His empathy and curiosity helped inspire me to embrace those things for myself. Even in moments when I encountered heavy memories and emotions, his presence and support served as a gentle, non-intrusive, yet strong tether. This ultimately allowed me to stay open to those thoughts and feelings long enough to begin understanding and shifting my relationship with them. During and since my session, what have long felt like sources of vulnerability in my life increasingly feel like sources of empowerment. Although I imagine each person’s psychedelic experience is highly unique, I would trust Dr. Canning to be the person alongside you for that journey.”

A one-time evaluation to determine whether IM ketamine treatment is safe and appropriate. Patients may pay the self-pay rate listed above or, when eligible, elect to bill insurance.

Insurance-billed screenings are submitted at Paradigm Mental Health & Wellness’s Usual & Customary rates, regardless of the self-pay tier selected. Patients who choose self-pay instead of billing available insurance must complete a waiver document.

The mandatory preparation session is self-pay and supports treatment readiness, intention-setting, education, safety planning, and therapeutic alignment. Additional preparation sessions may be scheduled as desired.

Medicine sessions and optional integration sessions are also self-pay. These services are not billed to insurance.

Choosing a reduced rate

Choose the tier that best reflects your financial circumstances using the eligibility descriptions above. Patients selecting Tier 2 or Tier 3 must complete a Self-Attestation of Financial Need.

Medicare and Medicare Advantage: This self-pay option is not available to Medicare or Medicare Advantage beneficiaries. Federal regulations governing Medicare-covered services prevent us from offering these self-pay services to these beneficiaries. Thank you for your understanding.

Frequently asked questions

What is ketamine?

Ketamine is a dissociative anesthetic that has been used safely for more than 50 years. Originally developed as an anesthetic, it is also widely used for pain management. More recently, research has demonstrated ketamine’s rapid-acting antidepressant effects, leading to its use for depression and other mental health conditions, particularly when conventional treatments have not been effective.

Ketamine works differently from traditional antidepressants. It affects the brain’s glutamate system and appears to enhance neuroplasticity—the brain’s ability to form and strengthen new connections. At the doses used for mental health treatment, ketamine can also produce a temporary non-ordinary state of consciousness that may offer new perspectives and facilitate emotional processing and insight.

What is IM ketamine treatment?

Intramuscular (IM) ketamine is ketamine given by an injection into a muscle. At Ketamine Wellness Services, treatment takes place in a supportive clinical setting with Dr. Canning present throughout the medicine session. Ketamine’s use for mental health conditions is off-label, meaning it is used for a purpose outside its FDA-approved indication.

Is ketamine a psychedelic?

Ketamine is not considered a classic psychedelic like psilocybin or LSD. It works differently in the brain and has been used as a medication for more than 50 years. However, at the doses used in IM ketamine treatment, it can produce experiences that share some features with psychedelics, including changes in perception, a reduced sense of self, feelings of connection or awe, and shifts in perspective.

Unlike classic psychedelics, ketamine has a relatively short duration of action, and the experience is often characterized by a sense of separation or distance from one’s usual thoughts, body, or surroundings—sometimes referred to as dissociation.

These experiences vary considerably from person to person and are not required for ketamine to be beneficial.

How is IM ketamine different from Spravato®?

Both IM ketamine and Spravato® are ketamine-based treatments used for certain mental health conditions, but they differ in their formulation, how they are administered, and the kind of treatment experience they can provide.

Spravato is an FDA-approved nasal spray containing esketamine, one of the two forms of ketamine. IM ketamine uses racemic ketamine, which contains both forms, and is administered by injection into the muscle.

Spravato is administered according to a standardized dosing protocol. IM ketamine allows dosing and the treatment experience to be more individually tailored.

IM administration produces a rapid onset and can produce a deeper, more immersive experience, including significant changes in perception, sense of self, and perspective. Our approach is designed around this experience, with individualized preparation, a carefully supported treatment setting, physician presence throughout the medicine session, and attention to integration afterward.

The two approaches are designed around different treatment models. Some people may prefer the structured Spravato protocol, while others may be drawn to IM ketamine because they are seeking a more individualized and experiential approach to treatment.

What might a treatment session feel like?

Experiences vary. Ketamine can temporarily change your sense of time, perception, and connection to your body or surroundings. Some people describe a dreamlike or inward-focused experience; emotions and memories may also arise. Preparation gives you an opportunity to discuss what to expect and how you will be supported.

How do you determine whether treatment is appropriate for me?

As a first step, we will send you a form with questions about your medical and mental health history. If no contraindications are identified during this initial review, we will offer you a medical screening appointment with Dr. Canning. At that appointment, you’ll discuss your health history, medications, and treatment goals to determine whether treatment is appropriate for you.

What does preparation involve?

A preparation session is required before your first medicine session. It supports treatment readiness, intention-setting, education, safety planning, and informed consent. Additional preparation sessions can be scheduled if desired.

How long will I be at the clinic?

Plan for approximately three hours for a medicine session. The total time varies according to your individual treatment and recovery needs, so allow flexibility in your schedule.

Can I drive after treatment?

No. You must arrange a ride home before your medicine session can proceed. You may not drive yourself home or drive for the rest of the treatment day.

How many sessions will I need, and how far apart are they?

The number and spacing of sessions are individualized. Some patients respond to a single session. Dr. Canning generally recommends an initial course of four to six medicine sessions, spaced at least one week apart, with the plan adjusted according to your response and needs. Results vary, and improvement is not guaranteed.

Do I need to work with a therapist or counselor?

Dr. Canning strongly encourages an established relationship with a therapist, counselor, or integration coach who can support your care and collaborate with him. Your support needs will be discussed during screening. Referrals to an integration specialist are available if needed.

What is integration?

Integration is the process of reflecting on your ketamine experience, making sense of it, and applying useful insights in daily life. It can involve discussion with a therapist or integration specialist and practical changes that support your goals. Optional integration sessions are available.

Can I be treated if I have Medicare?

Unfortunately, no. Ketamine Wellness Services cannot offer treatment to patients enrolled in Medicare or Medicare Advantage under our current self-pay model.

Medicare has specific rules governing physician billing and private self-pay arrangements. Under the requirements that apply to this practice, we cannot offer these services to Medicare beneficiaries simply by accepting payment out of pocket or having a patient sign a waiver.

We recognize that this can be disappointing, especially when you are seeking additional treatment options. This restriction reflects our practice’s billing requirements rather than a judgment about whether ketamine could be appropriate for you. Your usual clinician or health plan may be able to help you explore other treatment options and providers.

Does insurance cover treatment?

When eligible, patients may elect to bill insurance for the medical screening. Preparation, medicine sessions, and optional integration sessions are self-pay and are not billed to insurance. Reduced self-pay tiers are available based on financial circumstances. The self-pay option is not available to Medicare or Medicare Advantage beneficiaries. See the payment options above for details.
